There’s No Such Thing as a Simple Pelvic Health Patient: An Interview with Michelle Lyons
There’s no such thing as a simple pelvic health patient. Even the patient who walks in with “just a little stress incontinence” brings a history, a body, hormones, sleep, stress, previous injuries, bowel function, activity goals—and a whole lot more. And that's exactly why being a great pelvic health clinician requires so much more than memorizing protocols. We're joined by Michelle Lyons for one of those wide-ranging, thought-provoking conversations we always love having with her. Michelle has been part of PelviCon from the very beginning. She's our only 5-time PelviCon speaker, an incredible educator and one of the most thoughtful voices in our profession. In this episode, we talk about: 🪟 Why assessing the pelvic floor still matters. Your Business Doesn't Have to Grow
More revenue. More patients. More clinicians. More locations. We're constantly told that a "successful" business should always be getting bigger. But what if you don't actually want it to? 🤔 There are plenty of GREAT reasons not to grow. 👨‍👩‍👧‍👦 You're in a season where family needs more of you. ✈️ You want to travel or prioritize life outside the practice. 💰 You're already making enough money. 🙅‍♀️ You have absolutely zero desire to manage employees. ❤️ Or maybe you're just…happy with what you've built! And sometimes growth stalls for reasons you didn't choose—staff turnover, seasonality, capacity issues, losing a referral source, or just the normal ups and downs of running a business. A flat year doesn't mean you failed. But there's an important flip side… "I don't want to grow" can also become a really convenient story we tell ourselves when we're actually afraid to grow. 😬 There's a difference between intentionally choosing to maintain your business and avoiding the hard things required to take the next step. How to Actually Be an Evidence-Based Clinician: An Interview with Taryn Hallam
Reading research and actually understanding what it means for the patient in front of you are two VERY different things. And there may be nobody in pelvic health better at bridging that gap than Taryn Hallam. Taryn has essentially made research translation her full-time job—reading the enormous amount of pelvic health research being published and asking: What does this actually mean in the clinic? Because the abstract doesn't always tell the whole story. As Taryn puts it, the abstract is like the trailer for a movie. Sometimes the trailer accurately represents the movie. And sometimes...all the best parts were in the trailer. 😆 In this episode, we're diving deep into how we actually become evidence-based clinicians. We talk about: 📊 Statistical significance vs. clinical significance. A result can be completely “statistically significant” and still be basically meaningless to your patient. 🎬 Why you can't just read the abstract. What gets highlighted in an abstract isn't necessarily what matters most clinically. 🔬 Why one research paper is never the whole story. Taryn compares a single paper to opening a 450-page novel to page 210 and expecting to understand the plot. 🤰 The danger of extrapolating research to the wrong population. What happens when research on young, nulliparous athletes gets applied to a completely different patient sitting in front of us? 🤷‍♀️ Why “we don't know” is sometimes the most evidence-based answer. We don't need to pretend the research is more definitive than it actually is. 🧠 How experience changes our ability to assess. Can research showing poor reliability among relatively inexperienced clinicians really tell us what an expert with decades of pattern recognition can—or can't—feel? 🔥 And yes...we ask Taryn about the internal exam debate. Her answer? Let's just say she has some thoughts. 😆 We talk prolapse, stress urinary incontinence, levator avulsion, pelvic floor muscle training, tone, overactivity—and why Taryn believes an internal examination remains an essential component of best-practice pelvic health care. But she also makes an incredibly important distinction: Saying virtual care has value and saying an internal examination adds important clinical information are NOT contradictory statements. Both can be true. And sometimes our profession gets into arguments because we haven't even agreed on what question we're debating. 🎟️ Learn From Taryn at PelviCon 2026! Are We Really Providing Whole-Person Care If We Ignore Spirituality? With Sinéad Dufour
We talk a lot about providing “whole-person care.” But are we actually doing it? And if we leave spirituality completely out of the conversation...can we really call our care whole-person? This is exactly the kind of question we love exploring with Dr. Sinéad Dufour. Sinéad is one of the researchers and educators we respect most because she's constantly willing to examine the evidence, challenge her own assumptions and change her mind when the science takes her somewhere new. You Know His Research. But Do You Know Paul Hodges?
You might not immediately recognize the name Paul Hodges. But we can almost guarantee you've been taught his work. The 5 Levels of Care in Pelvic Health: What Is The Gold Standard?
Let's cut through the noise....If someone you loved needed pelvic floor therapy tomorrow, where would you send them? 🤔 Are there actually different levels of pelvic health care? We believe the answer is yes. The interesting part? Most pelvic therapists already act as though these different levels exist—we've just never had a common language to describe them. Think about it... If your spouse, your parent, or your child developed pelvic floor dysfunction tomorrow, who would you recommend? Most of us wouldn't say: ❌ "Just find any ortho PT." ❌ "Start with telehealth or try an online program." ❌ "Here's a great Instagram account." We'd recommend a pelvic floor specialist. That instinct reveals something important: we already recognize that different models of care have different capabilities. What Do We Actually Gain From an Internal Pelvic Floor Evaluation?
To be honest, we never imagined we'd need to explain this. Fair warning: this episode gets a little nerdy. Over the last two episodes, we've talked about why we believe internal pelvic floor evaluation remains the gold standard in pelvic rehabilitation. This week, we're answering the obvious follow-up question: What do we actually gain from performing one? For those of us who were trained to perform internal evaluations—and who use them every day in practice—the value seems almost self-evident. It's become such a fundamental part of how we think, reason, and treat that we rarely stop to ask why. It's also where our profession has focused its education, research, and clinical training for more than 40 years. But recent conversations in our field have challenged us to step back and articulate something we've largely taken for granted. So that's exactly what we do in this episode.
